Initial Complaint / Baseline Assessment

The patient is 45 years old, has had diabetes for 10 years, and maintains glycemic control with insulin. In 2021, the patient presented to our clinic with a necrotic, infected wound on the medial aspect of the left foot extending to the great toe, which had been present for 2 months (an opened amputation stump).

Identified Case

After the necessary surgical debridement, daily dressings were initiated with a solution and gel containing Polyhexanide + Poloxamer. In addition, electrical stimulation was administered once weekly. Healing was achieved with the treatment provided at the end of 1 month. No complications were observed.

Discussion and Conclusion

According to published Consensus data on wounds, Polyhexanide (PHMB) is indicated as the first option for wounds at risk of infection. Polyhexanide has high tissue compatibility because it selectively binds to acidic phospholipids found in pathogen cell membranes, particularly phosphatidylglycerol and diphosphatidylglycerol. The Polyhexanide+Poloxamer complex is rapidly attracted to the bacterial surface, binds to selective sites on the surface, and enables bacterial defense mechanisms to be neutralized. Polyhexanide reaches the bacterial cytoplasmic membrane, causing disruption of the cytoplasmic membrane and the release of intracellular fluid, electrolytes, and macromolecules outside the cell, thereby causing bacterial death. It has been shown to have a potent microbicidal effect against gram-positive and gram-negative bacteria, fungi, HPV, HIV, certain viruses, and amoebae.
In conclusion, considering its tissue compatibility, potent microbicidal effect, and ability to accelerate healing by providing a moist environment on the wound surface, it appears to be an effective treatment method for wound healing.
