Experience with Actolind® w Solution and Actolind® w Gel in a Traumatic Laceration and Pressure Wound in the Chin Region
Initial Complaint / Baseline Assessment
A 19-year-old male patient with only a known diagnosis of Obsessive-Compulsive Disorder. The patient was monitored in the ward for 6 days because he was COVID-19 positive, then admitted to our intensive care unit due to desaturation and the need for monitoring.
Identified Case
At admission: CRP: 86.7, Creatinine: 0.85, ALT: 25, AST: 44, Albumin: 38.6, Hemoglobin: 14.4. The patient was placed on oxygen support via a High Flow Nasal Cannula and treated with intermittent CPAP, plasmapheresis, and IVIG. Due to worsening oxygen saturation and arterial blood gases, he was intubated on day 8 of hospitalization and placed in the prone position for 16 hours. After being returned to the supine position, a 1*0.5 cm traumatic laceration caused by his tooth penetrating his lip and a 3 cm grade 2 pressure wound on his chin were observed. In the patient who remained in the prone position, the grade 2 pressure wound on the chin became necrotic during the third prone positioning. At this stage, daily dressings were performed with Actolind® w Solution and Actolind® w Gel (Polyhexanide+Poloxamer).
Discussion and Conclusion
On day 25 of hospitalization, the lesion below the patient’s lip had closed. The necrotic area on the chin had regressed to 2 cm. Debridement was performed. The patient, who was placed in the prone position four times for 16 hours during hospitalization, was extubated on day 14 of intubation. On day 35 of hospitalization, he was transferred to the COVID ward. At the time of transfer from the intensive care unit: CRP: 21, ALT: 40, AST: 18, Albumin: 34, Hemoglobin: 9.2, Creatinine: 0.59. The patient was followed after transfer to the ward and after discharge home; as of 19.01.2022, the lesions were observed to have healed.
